Registration 1. You may call into the surgery bringing with you your NHS medical cards from your previous NHS doctor. 2. You may call into the surgery without bringing with you your NHS medical cards from your previous NHS doctor. This may arise if you cannot find your cards or if you do not have any e.g. if you have recently arrived in the country. This is less preferable since it may take longer to trace your medical records and you will have to fill in more paperwork, so please find your medical cards if you can. 3. You may request to register online by completing the details on the next page. You will need to do this for each family member. When we receive your details we shall complete the paperwork for you and post it to you for your signature and return to us. Note that we cannot guarantee that we shall accept you as a patient since, for example, it may be that your address is not in our catchment area (click here to view our catchment area). If you request to register online and we send forms to you for signature then you will know that we are able to accept you, or if you attend the reception for registration in person the receptionist will be able to advise you. Important Note: Our list is "open" but we are very busy and would ideally like to have fewer patients, so sometimes we have no vacancies. We acknowledge that there is a shortage of GPs in Eastbourne and that new patients are allocated to us by the PCT (Primary Care Trust). Even when we have no vacancies we will usually take on relatives of our existing patients who live in our practice area and those people who live in very close proximity to our Surgery. Existing patients who live outside our practice area will be kept on our list but we would have to reconsider their registration if they move house. If you wish to register and we have agreed to take you onto our list, you will need to complete a New Patient Questionnaire which asks for details of your past medical history, etc. This form, together with a sample of your urine, needs to be brought to a New Patient Registration appointment with Kathy or Jeanette, our Health Care Assistants. From 1st April 2004 your registration will be with the Practice rather than with an individual doctor. We will normally allocate you within the Practice to one of the doctors taking on new patients, but if you have a wish to see a particular doctor you should put your request in writing to the Practice Manager. We will endeavour to meet your requests but some doctors in the Practice have too many patients and will be unable to take on any more.
